| Join Delta-Exchange: TransforMED's new collaborative network for practice transformation |
We learned from the National Demonstration Project (NDP)
that primary care practices want and need opportunities to communicate,
collaborate, and learn with and from other practices — which is why we
created Delta-Exchange — to bring together the PCP community around
tools and resources designed to help their practices transform to
patient-centered medical homes.
Delta-Exchange is an
easy-to-use, private, online collaborative Web site where primary care
practices can collaborate on projects, discuss issues, challenges and
successes, ask questions, share practical knowledge and experiences —
and work together to maintain the momentum for change and make the
transformation to a patient-centered medical home.

Report from the CEO:
The PCMH Code: It isn't a secret, but it is hard work |

| NATIONAL DEMONSTRATION PROJECT UPDATE |
Preliminary Answers to Policy-Relevant Questions
From the Early Analyses of the Independent Evaluation Team of the National Demonstration Project of TransforMED |
Preliminary Answers Part 1: Is
it easier for practices to provide chronic disease management if they
have a self-populating disease registry integrated within the EMR? What
are the primary challenges facing medical practices as they implement
EMR and other forms of HIT? Is implementing HIT enough to transform a
PC practice into a medical home? What type of support/resources, if
any, do practices need as they attempt to make transformative changes? Read Part 1 for preliminary answers to these questions and more »
Preliminary Answers Part 2: What
is the value of self-populating disease registries in chronic disease
management? What has been the experience of small practices trying to
implement an EHR and related technology? What is the concurrent issue
of quality of life for primary care physicians when they have
implemented the concepts of the patient centered model of care in their
practice? What do we know about patient engagement and empowerment in a
primary care practice making transformative changes? How long before we
might see outcomes? Read Part 2 for preliminary answers to these questions and more »

"How has your practice transformation journey
changed your leadership style?"
That's just one of the questions we asked the members of TransforMED's
Touchstone Group at the recent Touchstone Reunion learning
collaborative. The video responses are as inspiring, interesting and
varied as the physician leaders themselves. The Touchstone group is a
continuation of the collaboration between TransforMED and many of the
practices that participated in the Medical Home pilot National
Demonstration Project. See all the Touchstone Group Leadership Videos »
